Membership Guidelines:
- Cannot be combined with other insurance plans.
- Exclusively honored at Eastern Shore Dental Care.
- Expires 1-year after the date of initial enrollment.
- Must be paid in full at the time of enrollment.
- Not transferable to any other patient or family member.
- Cannot be combined with other discounts.
- Discounts cannot be used on services already rendered.
- All payments are due at the time of service.

- Adults = $325 per year *
- Children (13 & under) = $225 per year *
